Our COVID-19 Preparedness Plan
City, State

Clackamas Implant & Oral Surgery Center is committed to providing a safe and healthy workplace for all our employees. To ensure that, we have developed the following Preparedness Plan in response to the COVID-19 pandemic. Managers and employees are responsible for implementing this plan. Our goal is to mitigate the potential for transmission of COVID-19 in our workplaces and that requires full cooperation among employees and management. Only through this cooperative effort can we establish and maintain the safety and health of our employees and workplaces. Management and employees are responsible for implementing and complying with all aspects of this Preparedness Plan.

Our employees are our most important assets. We are serious about safety and health, and keeping our employees working. Employee involvement is essential in developing and implementing a successful COVID-19 Preparedness Plan. We have involved our employees in this process by listening and responding to concerns, and implementing their suggestions. Our Preparedness Plan follows Centers for Disease Control and Prevention (CDC) and local State guidelines and federal OSHA standards related to COVID-19 and addresses:
•  Hygiene and respiratory etiquette.
•  Engineering and administrative controls for physical distancing.
•  Housekeeping-cleaning, disinfecting and decontamination.
•  Prompt identification and isolation of sick persons.
•  Communications and training that will be provided to managers and employees.
•  Management and supervision necessary to ensure effective implementation of the plan.
•  Masks to be worn by all staff during direct patient care and cleaning processes, including set-up.
•  Mask worn when interacting with each other if physical distancing is not possible.
•  Dividers up in lab  and separation markers in the lunchroom.
•  Spreading out in the rooms to do notes away from each other.
•  Everyone wearing their own glasses.
•  3 ours between surgeries in specific rooms OR use of room filtrations units.
•  Surgical gowns laundered after every patient.
•  Scheduling to reduce total number of patients being seen at one time.
•  Patients and rides wait in their cars.
•  Screening patients when scheduling, when calling to remind about appointment, and upon check in.
•  Taking temperature and tracking respiratory symptoms for every employee at start of shift.
•  Wiping entire room after every patient, including the CBCT scanner.
•  Wiping all commonly touched surfaces every hour.
•  Scrubs and shoes to be left at work at end of day.

This Preparedness Plan has been certified by Clackamas Implant & Oral Surgery Center management and was posted throughout the workplace May 1, 2020. It will be updated, as necessary.
