Expert Surgical Care for Safe, Comfortable Tooth Removal

At Clackamas Implant & Oral Surgery Center, we understand that having a tooth removed can be a stressful experience. Whether your extraction is simple or complex, Brett M. Sullivan, DMD, MD, a dual-degreed oral and maxillofacial surgeon, delivers care with precision, comfort, and safety at the forefront.

Why Are Teeth Extracted?

Tooth extraction may be recommended when:

  • There is severe decay or infection that cannot be restored
  • Periodontal disease has caused significant bone loss
  • The tooth is fractured or broken beyond repair
  • Teeth are impacted, poorly positioned, or contributing to crowding (including wisdom teeth)
  • A non-functional tooth needs to be replaced with an implant, bridge, or denture

Types of Extractions We Perform

Simple Extraction

Used when the tooth is visible and easily accessible. The tooth is gently loosened and removed under local anesthesia. Ideal for routine cases involving minimal surgical intervention.

Surgical Extraction

Required for teeth that are:

  • Impacted or partially erupted
  • Fractured below the gumline
  • Surrounded by dense or uneven bone

Dr. Sullivan makes a small incision to access and remove the tooth, often under IV sedation or general anesthesia for enhanced comfort.

What to Expect

1. Consultation & Imaging

Your care begins with a comprehensive exam and 3D cone beam imaging (CBCT) to assess the tooth’s position, surrounding bone, and any potential complications.

2. Anesthesia & Sedation

We offer local anesthesia, nitrous oxide, oral sedation, and general anesthesia based on your needs, anxiety level, and medical history. Your comfort and safety are our priority.

3. Surgical Procedure

The tooth is gently removed using advanced surgical techniques. If needed, Dr. Sullivan may:

  • Section the tooth to aid removal
  • Contour the bone to support healing
  • Place sutures for soft tissue closure

4. Post-Op Care

You’ll leave with detailed written recovery instructions, gauze to manage bleeding, and guidance on pain control, hygiene, and diet.

Recovery Timeline & Tips

  • Initial healing: 1–2 weeks for gum tissue
  • Light activity: Usually resumed within 24–48 hours
  • Full bone healing: May take several months (especially before implant placement)

For a Smooth Recovery:

  • Avoid smoking and drinking through straws (prevents dry socket)
  • Stick to soft, cool foods like yogurt, scrambled eggs, and smoothies
  • Take medications as prescribed
  • Avoid heavy lifting or exercise for the first 48 hours

Understanding Dry Socket

The good news is that of the thousands of dental extractions performed by Dr. Sullivan, he has had less than 10 cases of Dry Socket occur.

Dry Socket occurs when the blood clot protecting the surgical site becomes dislodged, exposing the bone and nerves. It can delay healing and cause pain.

Preventive Tips:

  • Do not use straws
  • Avoid tobacco products
  • Follow all aftercare instructions closely

Tooth Replacement Options

Leaving a space after extraction can lead to shifting teeth, bone loss, and bite misalignment. Dr. Sullivan will discuss long-term options such as:

  • Dental Implants – Permanent, functional tooth replacements
  • Fixed Bridges – Non-removable options that restore missing teeth
  • Removable Dentures – Suitable for multiple missing teeth or full arch restoration

Planning for replacement during the extraction process can save time and improve final outcomes.
